How do I take the survey?

Once you have accessed the site, then you can enter your purchase information on the site. This will include the following info from your most recent receipt:

• Date of purchase
• Time of purchase
• Store number
• Register number
• Transaction number

When you access the survey site, you will see a sample of a Helzberg receipt to make it easier to gather your info. Once you have entered the info on the survey site page, you will then hit the start button.

From here you will be taken to the survey questions. The amount of time to complete the survey should be no longer than 15 minutes. Once you are completely finished, you will be entered to win a $500 prize.
