Captain D’s Survey

Do you love to eat Captain D’s? Did you have any challenges during your last visit? Then, feel free to visit the company online survey site. The site is available to gain information about your personal dining, or drive through experience. They would love to hear from you!

How do I get started with the survey?

First of all, you will type the address into your internet address bar provided below. Then, you will hit enter – this will take your directly to the survey site.

You will know that you have accessed the proper survey site. You will see the Captain D’s logo in the upper left hand corner of the webpage. You will need a few pieces of information to start the survey process.

On your most recent restaurant receipt, you will see a list of numbers at the bottom of the paper. It will say “Survey Code” above these numbers. You must enter all three lines of numbers into the webpage for the survey. Then you can click the start button to begin.

You will be given a validation code when you are finished with the survey completion. The code can be used on your next visit to a nearby Captain Ds. Just hold on to your receipt and take it with you next time you decide to go.

Kid’s Birthday Club

Captain D’s offers a Kids Birthday Club for those who are age 12 and under. If you are interested, then you can enter the information online which makes it quick and simple.

All you need to enter online is your child’s first name, email address, birthdate and favorite location. Your child will receive a coupon for a free kid’s meal for their birthday.

Kid’s Day at Some Locations

Captain D’s loves kids and many of their store locations offer Kid’s Day on Thursdays. Kids will eat free with the purchase of an adult entrée. This is available for dine in only and participating stores may vary. So, if you have children who love Captain D’s, then why not see if your closest location offers Kid’s Day.

Senior Day at Some Locations

A variety of Captain D’s locations offer Super Senior Day. This special promotion is available to members who are age 62 and over. They have the option to choose one of eight meals for $4.99 or less. Participating restaurants offer Senior Day on Sundays, and Wednesdays.
